iUnite is a web portal where agencies can complete Healthy Aging Grant Applications and Outcome Reports. Healthy Aging Grant Applications must be completed and submitted electronically through iUnite at https://www.iunite.ca/. Once logged in, you can find Grant Applications and Outcome Reports under the Agency Submissions tab in the navigation toolbar at the top of the page. Data does not need to be migrated from your database or data tracking method to submit a Grant Application or Outcome Report.
This form is for new agencies that do not yet have an iUnite account. If your organization held one of the following Healthy Aging grants in the last 3 years, then your agency already has an iUnite account:
- Better at Home
- Family & Friend Caregiver Supports
- Social Prescribing
- Therapeutic Activation Programs for Seniors (TAPS)
If your agency holds one of the above grants, please reach out to an Admin User at your agency and they will be able to create an account for you. Users can be added in iUnite under the “Users” tab. Here is a link to guide that may be helpful: iUnite User Management Video Guide.
